Booking Ongoing Stays at Myka Place

Once your induction and consent to services have been completed, you’ll be able to use the link below to book ongoing stays at Myka Place with ease.
 
Getting Started – Required Documentation
 
To ensure we deliver safe, consistent, and individualised care, we ask families to provide the following information prior to commencing services at Myka Place.
 
NDIS & Service Documentation

  • A copy of your child’s NDIS plan (including evidence of funded supports such as respite, if applicable)
  • We will then provide:
  • A formal quote
  • Service Agreement
  • Required consents
 
Health, Medical & Care Plans

Copies of all current and relevant care plans, including but not limited to:
  • Seizure Management Plans
  • Behaviour Support Plans
  • Feeding and nutrition plans
  • Manual handling and positioning plans
  • Airway management profiles, including tracheostomy care where applicable
  • Health and medical management plans
  • Any recent correspondence from treating medical practitioners or specialists
 
These documents ensure our team can safely support complex needs while maintaining best practice and clinical governance.
 
Daily Routines & Individual Needs

  • Information about your child’s:
  • Daily routines and preferences
  • Communication methods
  • Sensory needs, comforts, and triggers
 
We will also ask you guided questions to gain a thorough understanding of your child’s needs, strengths, and support requirements.
 
Medications

  • A current medication list, including:
  • Photos of medication bottles, boxes, or scripts clearly showing:
  • Medication name
  • Dose and strength
  • Timing and method of administration
 
This allows us to complete accurate medication charts and ensures medications are administered safely and correctly.
 
Clinical Review & Individualised Care Planning
 
Once all required documentation is received:
  • Our Clinical Nurse will review the information with you
  • Individualised care plans will be developed specific to the supports being delivered at Myka Place
  • Home routines will be maintained wherever possible, ensuring smooth, familiar transitions between home and Myka Place — and back home again.
 
Additional Activities & Consents
 
For optional activities such as:
  • Swimming
  • Community outings
  • Transport
 
Additional activity-specific consent forms will be provided prior your admission.
